The law gives you many rights regarding your health information and we will properly respect the privacy of that information. We will not make any use or disclosure of your health information unless you sign a written "authorization form". We respect our legat obligation to keep health information that identifies you private. A copy of our privacy pratices is given to each patient and always avialable at our office.
